Fixed not pausing on monitor exit if entered whilst paused. Only re-pauses for ‘x’ and ‘g’ commands.

Fixed PC displayed as $0000 on entering monitor while paused. One known issue of this implementation is that pause is force disabled when entering the monitor and before executing any monitor command. This is because pause needs to be disabled while stepping through instructions, and this is the simplest thing to do for now just before release.
